Childbirth is one of the most widely shared experiences of roughly half the world?s population. Yet even among the most highly developed countries, the quality of that experience varies tremendously, with high-intervention practices becoming increasingly the rule in the US, while countries such as the Netherlands have relatively low numbers of cesareans, but high numbers of homebirths with excellent results. A discourse of medical rationality would have us expect that wealthy countries provide the best care available for whatever health needs present themselves, including the needs of women giving birth. Yet countries clearly differ in what they consider the “best” care. Why do otherwise similar countries diverge in their approaches to childbirth and maternal health? This paper documents the extent of variation in obstetric practices and maternal and child health outcomes among OECD and EU countries and then presents research design and preliminary findings for a new multi-country investigation into policy divergence in maternal health policies.

With prescription drugs being for years now, known as the 4th leading cause of death in the US; properly prescribed; how can they claim in fact that 45,000 will die if not treated by medical care? With medical mistakes as well being classed as the third leading cause; these medical field studies are about as worthless as it gets. Undoubtedly in certain circumstances medical care can saves lives; but in the long run as fact, it has done more harm than good.

10 to 25% of public procurement spending (including on pharmaceuticals) is lost to corrupt practices.
In developed countries, fraud and abuse in health care has been estimated to cost individual governments as much as US$23 billion per year.
Countries with a higher incidence of corruption have higher child mortality rates.
Lack of medicines and counterfeit and substandard medicines lead to patient suffering and have direct life or death consequences.
To reduce corruption, thorough checks and balances are required at each step in the medicine chain. Good governance includes transparency, accountability, promoting institutional integrity and moral leadership.
Unethical practices in the medicines chain
The medicines chain refers to the steps required for the creation, regulation, management and consumption of pharmaceuticals. Corruption in the pharmaceutical sector occurs throughout all stages of the medicines chain, from research and development to dispensing and promotion1.
Unethical practices along the chain can take many forms such as falsification of evidence, mismanagement of conflict of interest, or bribery. The Figure illustrates key steps of the medicines chain and some examples of unethical practices.
Good governance within the medicines chain is one essential means for optimizing public health outcomes. For example, countries with higher incidence of corruption have higher child mortality rates, even after statistically controlling for quality of health-care provision 2.
